
Your First Visit
It's not uncommon for patients to feel timid about their first physical therapy session. Don't worry! We will do our best to make you feel at ease. Your therapist will take the time to help you understand every aspect of treatment. Our friendly and knowledgeable office staff will help you cruise through the paperwork and the insurance maze.
What do I need to bring?
The office staff will need the referral/prescription that was provided by your doctor, your insurance card and a picture ID. If you'd like to get a head start on the FUN task of filling out forms, we've provided the following for your convenience. Please click on the link to print forms.

What do I need to wear?
For the therapy session, wear comfortable clothing or bring gym shorts or sweats. Don’t worry if you forget shorts, we can provide you with a pair.
How long will my session last?
Expect the first visit to take at least one hour. This provides our therapist with enough time to gather as much information about your injury or condition to develop the best plan of action. Follow-up visits will last from 45 minutes to one hour.
Do you take my insurance?
Ultimately, it is your responsibility to know your insurance coverage for therapy, including your deductible and copayment. We will do insurance verification and file your insurance for you, and we will be glad to help you understand how your insurance works. Eagle Rehab is a preferred provider for the following plans:

Can I purchase equipment at your clinic?
We do offer a variety of therapeutic products such as pulleys, ice packs, biofreeze, physioballs, shoe inserts (powersteps and superfeet), power cables and thera-band.
